The method, which was still in its infancy at the time, is now the standard worldwide<\/em><br><br>For a woman suffering from <strong>breast cancer<\/strong>, the loss of a breast is not just a physical wound \u2013 it deeply affects her self-image and everyday self-confidence. Reconstruction is therefore not just an aesthetic procedure, but an important step on the path to <strong>emotional<\/strong> and <strong>physical healing.<\/strong><br>Exactly three decades ago, plastic surgeon<a href=\"https:\/\/www.priora.eu\/en\/about-us\/our-team\/milomir-ninkovic\/\" data-type=\"team\" data-id=\"325\"> Prof. Milomir Ninkovi\u0107<\/a>, PhD, performed the first <strong>breast reconstruction<\/strong> using vascular flaps from the lower abdomen. The method, which was still in its infancy at the time, is now the standard worldwide. The beginnings, however, were not simple.<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img loading=\"lazy\" decoding=\"async\" width=\"660\" height=\"660\" src=\"https:\/\/www.priora.eu\/wp-content\/uploads\/2024\/03\/Kirurgija-dojke.jpg\" alt=\"Breast surgery at IMC Priora\" class=\"wp-image-2170\" srcset=\"https:\/\/www.priora.eu\/wp-content\/uploads\/2024\/03\/Kirurgija-dojke.jpg 660w, https:\/\/www.priora.eu\/wp-content\/uploads\/2024\/03\/Kirurgija-dojke-300x300.jpg 300w, https:\/\/www.priora.eu\/wp-content\/uploads\/2024\/03\/Kirurgija-dojke-150x150.jpg 150w\" sizes=\"auto, (max-width: 660px) 100vw, 660px\" \/><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\">Breast reconstruction<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Throughout the history of <a href=\"https:\/\/www.priora.eu\/en\/departments\/plastic-esthetic-reconstructive-and-hand-surgery\/\">reconstructive and plastic surgery,<\/a> there has been a tendency towards breast reconstruction, only the methods have changed. With the introduction of <strong>microsurgical<\/strong> methods in the 1980s, related to the so-called post-traumatic surgery, the technique of microsurgical tissue transfer began to be applied in breast reconstruction as well \u2013 begins <strong>Professor Milomir Ninkovi\u0107, Head of the Department of <a href=\"https:\/\/www.priora.eu\/en\/departments\/plastic-esthetic-reconstructive-and-hand-surgery\/plastic-surgery\/\" data-type=\"page\" data-id=\"266\">Plastic, Reconstructive, Aesthetic Surgery and Hand Surgery<\/a> at the <a href=\"https:\/\/www.priora.eu\/en\/\" data-type=\"page\" data-id=\"91\">Priora International Medical Center<\/a> in \u010cepin.<\/strong><br><br>He also recalls the <strong>Swedish<\/strong> plastic surgeon <strong>Hans Holmstr\u00f6m<\/strong>, who in the late 1970s published the results of a new technique for <strong>breast reconstruction<\/strong> using tissue from the lower abdomen.<br><br>&#8211; <strong>The idea<\/strong> for this venture was given to him by a patient who commented that it was a shame that, after aesthetic procedures for <strong>tummy tucks<\/strong>, all the excess tissue was thrown away. <strong>Wouldn&#8217;t it be useful<\/strong>, she asked him, to use it for breast reconstruction? It has the same consistency, as well as enough skin. <strong>Holmstr\u00f6m<\/strong> thought about it and performed two operations using the tied <strong>flap method<\/strong>. He transferred tissue from the lower abdomen, through the muscle as a carrier of blood vessels, to the breast area, pulling it through a skin tunnel. He aroused enormous interest among fellow <strong>plastic surgeons<\/strong>, and this technique was particularly intensively used in Atlanta, USA \u2013 says <strong>Dr. Ninkovi\u0107<\/strong>, who knew <strong>Dr. Holmstr\u00f6m<\/strong> personally and they talked about it a lot.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Breast reconstruction &#8211; Dr. Milomir Ninkovi\u0107<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The famous <strong>plastic<\/strong> surgeon <strong>Michael Scheflan<\/strong> from <strong>Atlanta<\/strong> visited Ljubljana in the mid-80s, where he also met a young colleague from <strong>Sarajevo<\/strong>, <strong>Dr. Milomir Ninkovi\u0107.<\/strong><br><br>&#8211; At the medical <strong>congress<\/strong>, he demonstrated the technique of breast reconstruction, so we asked him to perform such an operation together in Ljubljana. That operation was not so successful, a large part of the tissue failed. Then the <strong>Ljubljana<\/strong> plastic surgeon <strong>Zoran Arne\u017e<\/strong> decided to apply microsurgical methods: he completely separated the tissue from the lower part of the abdomen, leaving it on one blood vessel, and then transplanted it to the breast area &#8211; recalls <strong>Milomir Ninkovi\u0107.<\/strong><br><br>This is how the flap developed, which was initially transmitted in <strong>combination<\/strong> with skin and muscle. After <strong>Dr. Ninkovi\u0107<\/strong> joined the <strong>Clinic for Plastic and Reconstructive Surgery at the Medical University of Innsbruck<\/strong> in the fall of 1992, he devoted himself, among other things, to breast reconstruction using microvascular flaps.<br><br>Not everything went <strong>smoothly<\/strong> there either. The problem arose \u2013 to which blood vessel should the flap taken from the abdomen be tied? The answer was provided by an <strong>anatomical study<\/strong> and clinical application of the results of the anatomical study, which was led by <strong>Professor Ninkovi\u0107<\/strong> and his colleagues from the<strong> Clinic for Plastic Surgery in Innsbruck<\/strong>, in collaboration with the Anatomical Institute. It was published in <strong>1995 in the British Journal of Plastic Surgery<\/strong>. The study showed how this problem could be overcome by connecting the flaps from the abdomen to a blood vessel located directly behind the ribs, which <strong>cardiologists<\/strong> normally use for heart bypass surgery.<br><br><strong>Professor Ninkovi\u0107<\/strong> has been working on the same principle for 30 years, which he developed together with his <strong>Austrian colleagues<\/strong>. He has performed hundreds of such procedures, with which he has restored not only the shape of <strong>his body,<\/strong> but also faith in himself and new hope for a better future for his patients. Each <strong>reconstructed breast<\/strong> tells a story of courage, struggle and victory.<br><br>&#8211; There are <strong>many places<\/strong> on the body from which tissue for breast reconstruction can be taken &#8211; from the lower abdomen, the inner thigh, the upper or lower parts of the buttocks&#8230; All that is <strong>important<\/strong> is the <strong>microsurgical<\/strong> ability to transfer that tissue from one place to another and connect it &#8211; he says. Such procedures take four to five hours, and are technically quite complex.<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img loading=\"lazy\" decoding=\"async\" width=\"1024\" height=\"683\" src=\"https:\/\/www.priora.eu\/wp-content\/uploads\/2024\/03\/Dr.-Milomir-Ninkovic-Priora-news-1.jpg\" alt=\"An elderly man in a white doctor's coat stands in front of a modern building with the sign 'PRIORA' above the entrance.\" class=\"wp-image-2212\" srcset=\"https:\/\/www.priora.eu\/wp-content\/uploads\/2024\/03\/Dr.-Milomir-Ninkovic-Priora-news-1.jpg 1024w, https:\/\/www.priora.eu\/wp-content\/uploads\/2024\/03\/Dr.-Milomir-Ninkovic-Priora-news-1-300x200.jpg 300w, https:\/\/www.priora.eu\/wp-content\/uploads\/2024\/03\/Dr.-Milomir-Ninkovic-Priora-news-1-768x512.jpg 768w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">One of the current methods of <strong>breast reconstruction<\/strong> after <strong>cancer surgery<\/strong> is placing an implant in a preserved skin \u201c<strong>pocket\u201d.<\/strong><br><br>&#8211; Each <strong>implant<\/strong> has a limited lifespan, and if the <strong>patient<\/strong> reacts to it as if it were a foreign body, capsular fibrosis can develop in <strong>15 to 30 percent of patients<\/strong>, especially in the case of subsequent radiation. Reconstruction with the <strong>patient\u2019s own fatty tissue,<\/strong> connected to the skin, provides a result that lasts a lifetime, and the breast has a normal consistency and color \u2013 concludes <strong>Prof. Milomir Ninkovi\u0107, PhD.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Breast reconstruction is one of the most complex procedures that affects the self-confidence and mental health of every woman.
